Below you will find all the medical forms needed for your child to attend camp:
State law requires that all campers have the Health Form completed by a physician prior to camp, which includes a physical exam within the past 36 months. The Health Form is required on file two weeks prior to the start of their camp session. If the Health Form is not on file two weeks prior to the start of your camper’s session, the camper’s spot will be forfeited.
Health Form 2017: This form is required to be filled out by a physician.
KI Form 2017: The Potassium Iodide Child Medication Authorization Form is required to be on hand. If you register online, this form is integrated in as a question and no need to fill out the paper form.
Authorization to Administer Medication 2017: The Authorization for Administration of Medications is required for any prescription or over the counter medication to be administered during the camp day.
Non-Prescription Topical Medication 2017: This form is required to give camp staff permission to apply suncscreen and/or bug spray.
Permission To Treat Form: This is required to give camp staff permission to adminster first aid if necessary and provides contact information. This is no longer a separate form. It is now included in the Parent Authorization & Permission to Treat section #13 on the Registration form.
All necessary paperwork must be on file two weeks prior to the start of your camper’s session. Campers cannot stay at camp without a Health Form with physical on file.
